What Does “Globally Inclusive to Surgery” Mean?

Whenever a patient undergoes a surgery, insurance considers that surgery as one full package. This package includes not only the surgery, but also certain related services performed before and after the surgery.

This is called the Global Surgery Package.

So, when another service is billed during the global period, the payer may deny it saying:

This service is already included in the original surgery payment.

3 Time Periods in Any Surgery

Every surgery has 3 phases:

1) Pre-operative Period

2) Intra-operative Period

If the service is related to the main surgery:

It is generally included in the global package
Payment will not be made
Modifier may not work unless allowed

“May I get the denial date?”
“When was the main surgery performed?”
“What is the global period for this surgery?”

Step 2: Calculate Global Period Range

Global period begins from main surgery date.

Surgery Date: Jan 1
Global Period: 90 days
Global Period End Date: April 1 (approx.)

Now check:

Does the denied service DOS fall within this range?

Call payer and request:

“Please reprocess the claim as DOS lies outside the global period.”

Then ask:

“What is the TAT for reprocessing?”
“May I get the claim number and call reference number?”

Set follow-up based on the given TAT.
